Track spontaneously appearing twice in album

Very occasionally a track will appear twice in an album. Is it just me? I played the album earlier today and it only appeared once then. To add to the confusion now it appears three times in the playlist.

Since nobody has commented it must just be me but this has happened again. No scan involved, just playing tracks. The path to each duplicated track is exactly the same.

I have seen it from time to time (2 or 3 times) but when I have done a full clear and rescan they have disappeared.

Yup clear and rescan fixes it but how does it happen. I examined the library.db database and the four entries all have unique IDs and timestamps but the three new ones appeared out of nowhere. Something is causing them to be added to the database.

I am sure I saw another thread about the same issue very recently but can't find it now.

This seems to happening quite frequently. I don't remember it happening in the dim and distant past but now it is happening enough to be really annoying.

This is getting annoying. Yet another track appearing twice on an album for no obvious reason. This normally seems to happen after a track has been played, no scan involved.

How often do you reckon this occurs? I've never seen this (or, more correctly, noticed this), and I use LMS almost continuously as an enforced homeworker.
    LMS 8.1.2, Perl is 5.32.0

What 3rd party plugin are you using? Anything like a mixer plugin?

Would you mind sharing your library.db and server.log with me?
