That would be an interesting finding. I'm seeing those scans all the
time, but just double checked again and didn't find a single duplicate
    in my database.
I have had automatic online scanning disabled for two weeks now and have not seen any duplicate tracks appearing. Not really proof of anything but food for thought. If I enable it again which logging settings should I use?

Can you share that log, and what you consider "concerning importers"?

    I already shared it here

I seem to remember seeing a lot of lines in the log mentioning finding "importers" after LMS restarts.

Well this is weird. I have just noticed these duplicated tracks. In library.db they both have the same timestamp back in 2013. I have another copy of library.db from April this year and the duplicate didn't exist then. How can that happen?

You keep getting these don't you?
I'd suggest that what is happening may well be related to a disk reading error at scan time. What are you hosting your LMS on and where are the music files stored?

This is different though. The other duplicates had a different timestamp as you would expect. This one didn't.
The server is a Pi4 with a USB attached 3TB drive.

Timestamp is the wrong place to look apparently. The time added for the duplicate was 17-05-21 1:48:57 PM and the original was
16-05-21 11:28:25 PM. This is the first time it has happened since I turned off automatic updating of my online music integration weeks ago.

This album hasn't even been played and the first track appears 15 times

    Track spontaneously appearingtwice in album

    > This album hasn't even been played and the first track appears 15 times

I'm pretty sure it's a problem in your setup. Disable all 3rd party
plugins and see whether you can reproduce the issue after a full rescan.
If that doesn't show the problem, add plugins back. MIP in particular I
would consider a good candidate to cause issues.

Every time a duplicate track is added it is after LMS is restarted. MusicIP plugin isn't installed just MIPmixer.
