I have to assume that the duplicate tracks always appear after playing tracks that I often return to otherwise I wouldn't notice the issue at all. As far as I can tell the duplicated tracks only appear for tracks that I have recently played.

How would you play them? Go back to the album and pick it?

I play them from the album using Material Skin.

Michael this happened again. Dreamer appears three times in this image. I examined the database and found the extra entries were added at
    Monday, April 12, 2021 7:24:23 PM and
    Monday, April 12, 2021 7:46:33 PM

Looking at the server log the server was restarted at these times followed by a scan. I have attached the log. This must be getting closer to the reason.

It seems to me that the track appearing multiple times is the last local track played before an automatic scan takes place for some unknown reason. Maybe the online music scan. I have disabled that now, let's see what happens

That would be an interesting finding. I'm seeing those scans all the
time, but just double checked again and didn't find a single duplicate
    in my database.

The wierd thing is it isn't happening every time an online scan takes place. If a scan takes place after an LMS restart would that be an online scan? It is hard to tell from the server log.

If there's no pass for local files, then it's either a playlist only or
online scan. But what would trigger the scan right after the restart? I
sometimes restart LMS dozens of times a day without the scan kicking in
    immediately.

No idea but you can see it twice in the server log.

Actually I misread the server log. There is no scan at LMS restart just a lot of lines concerning importers. Nevertheless it was during this start up phase that the duplicate tracks were added.
