oh, sorry email address and password. LMS UI->settings->mysqueezebox.com (tab)-> reenter the email address and password, remember to hit apply. A few here were helping another member, tried everything, but this worked. Kind of a stab in the dark.
Since you are running LMS on windows, best tp make sure you are running it as an administrator to eliminate that. I run it on a pi, but I have been reading where recent windows account security updates have cause issues with things that previously ran. Try running it as an administrator, even if you are logged in as an administrator (sounds goofy 'eh?)

I am sorry I don't have an answer for you, and it seems to me that you are doing all the right tests/trials. I am posting because this issue is reported and discussed here allot with all sorts of resolutions. If you do a google search of, say "lms cannot connect to mysqueezebox.com" like this:

Quick question - I noticed that my firewall has LMS allowed for inbound rules but not for outbound. Would that make a difference?

I have the same setting
On the LMS Control Panel > Diagnostics do all the connections report OK ?
Under the Advanced tab the is a button to clear the Network cache
When one uninstalls LMS it often leaves behind some files, these are best removed before re-installation as any corrupt setting will just be reinstated.
LMS cannot connect to mysqueezebox.com. My player is able to login to it, and I can login to it using the website, but LMS cannot login to it. I have been able to download the apps that I require (Qobuz/Spotty) but BBC Sounds will not work (and I love the app).
I have turned off my anti-virus (Windows Security) and I have turned off my Windows firewall and done a full re-install. I get the message (through LMS) that I have an invalid username or password on mysqueezebox.com and the same on the BBC Sounds app. Even with the virus and firewall off, it will not connect (LMS is an allowed app on the Firewall).
