Please refresh my memory on adding Plugins

Been using this software for a very long time, but it's been a while since I've added any plugins and I have some basic questions:

1. Do all the approved plugins automatically update?
2. It seems like a lot of plugins don't have a repository that you can include in the list of 3rd party plugins repository list, so
3. When no repository is listed, do you just un-zip the files for the plugin in the plugin directory?

Auto-update is optional, but applies to them all. And I wouldn't know of many plugins which aren't in the list already. Would you have an example?

No, I got confused because I could not find C-3PO's upsampler. It's there now.
