Exact Audio Copy has an option to rip a CD and create a cue sheet.
I don't have a non-Windows solution, but I'm sure others can help
you out if required.

I would be interested to know about this on a Mac, OS X Panther. I
looked at MacFlac and (the slightly newer) xACT. I could be missing
something, but I couldn't see how to rip a CD.
