So I should blow away my playlist (.m3u files) that SlimServer created and
the issue may be solved?

Yes, definately.

Uninstall only gets rid of files created during the install. History and
current playists (__history.m3u, __00_04_20_00_00_01.m3u for example) will be
created automatically after you run teh server. It could be windows related,
but most likely tied to these files. Getting rid of them should help.
