I noticed that the 6.5.1 version of SlimServer requires a MySQL client
module ... how important is this? And how much of the schema is MySQL
specific? Would it be easy to port to PostgreSQL?

I'll look into it more, but would prefer to be able to use SlimServer
with a database I already have installed and working rather than add
another.

That was my first thought as well, but today I don't know whether it is
worth the effort: today I don't care too much about where SlimServer is
keeping it's data. It's read-only, mainly a cache of data stored
elsewhere. I do not back it up, as I don't care rebuilding the DB from the
tags. In the end I consider it a black box to me.

SlimServer handles running its own MySQL instance pretty well, so I don't
have to care about this neither. I'm happily running it alongside my main
MySQL server (which is too old for SlimServer). The additional 10-20MB of
RAM "waisted" are the only downside I see, but it's negligible in my
configuration (512MB RAM).
