On OS X, it is
~/Library/SlimDevices/slimserver.pref

The playlist directory is actually defined in the slimserver prefs, or
inside the server setup screen of the web interface. For example, mine is

~/Music/playlists/

Mine is mostly empty since I use iTunes. In fact, I wouldn't bother
copying mine.

If you have a large library, this can take some time. I am impatient and
want to hear music instantly, so I keep this one too. It is located at:
~/Library/Caches/SlimServer/

If you have installed plugins, you may want to grab the entire "pref pane"
to preserve them as well, since re-installing slim server will blow them
away. The location depends on whether you installed it for yourself only
of system-wide.
It is either in :
~/Library/PreferencePanes/SlimServer.prefPane

or
/Library/PreferencePanes/SlimServer.prefPane

That depends on where the plugins were installed. Those places are
certainly possible homes for them. But a better place, precisely
because they don't get lost on a re-installation or upgrade, is in

/Library/SlimDevices/Plugins (or ~/Library ...)
