headphone detection is only working for the minority I guess.
Search for the Headphone Switcher plugin/applet to manually switch to
headphone out.
You will find plenty of threads about the headphones feature (and all
its problems and workarounds) if you search this forum.

Headphone detection works quite well for me. Some things I have found that have to be considered for it to function properly:
When connecting headphones, make sure the Controller is in stopped state (not just paused). Same thing when disconnecting, don't go pulling the plug out while it's playing, but stop the playback by holding the pause button for a second or so. Another way to ensure it's stopped is to clear the playlist.
