Regardless of how long I leave the controller in the charging cradle, after about 5 minutes it says PLEASE CHARGE ME, and will not let me do anything else. Is it overcharged? Can this be avoided? Is my battery finally dying? If it is, how can I replace it? Thank you.

Yep they last a little longer than a year, if you use the controller a lot.
If still under warranty you can contact support and get a new one free.
Or shop one on www.logitech.com

I just wrote to Support regarding my pretty much dead Controller battery. Within hours, I received a response where they told me a new one is on the way. I think that's great service. For many devices, the battery warranty is shorter than for the device itself.
After the warranty, however, it seems the cost of a new battery each year has to be considered as an expense for the convenience of the Controller. The price should however be more reasonable. It's a battery!
