I just setup the Squeezebox and it is equipped with the following:
Server 5.4.0
Player Firmware Version 40
Save Playlist Version 1.0
In order to create a playlist using the remote, I hit ADD on the remote and
save the currently playing file or radio station; I then shoot over to
Plugins/Save Playlist, enter a playlist name, hit right arrow (then again),
the display indicates it is saving it on the server (even the correct subdir
is displayed) but the file does not save:
Previous message I found below indicates this was likely fixed - but
apparently not?
Anyone have a fix for this?

If you are referring to the Save Playlist from teh Player UI, then this bug
has
been fixed in today's nightly. A small change in the code that writes
playlists broke the Save Playlist and Bookmark plugins.

The fix will not be in the 5.4.0 release, but should be in the latest nightly
build for 5.4.1 found here:
http://www.slimdevices.com/downloads.../latest/5.4.1/
